At this time we accept patients with HMO insurance that have Physician Associates/Health Care Partners and Crown City Medical Group as their IPA.

We Accept PPO insurance but we do not bill for PPO insurance, as coverage for PPO insurance will depend on your particular plan’s benefit package. Every PPO insurance plan is different with different coverage benefits, deductibles and out-of-network coverage.

If you have a PPO, we will present you with a paid cash receipt and a billing form for our program and services so that you can submit to your insurance plan for your reimbursement.

Non-Insured Patients

Patients who are uninsured will be charged our Usual and Customary fee for all our programs and services.

Patient Assistance Programs (PAP)

At Diabetes Care Pharmacy & Health Program Centers we believe that health education should be a right and not only for the privileged. If you are a patient with financial hardships, you may qualify for our Patient Assistance Program that affords a partial scholarship for our programs without the financial burden.
